Executive Function Check-In
A quick, informal look at how a student manages daily tasks — not a test, just something to help us understand how to support them well from day one.
For the student’s parent(s): Please check any behavior below that happens more often than you’d expect for your child’s age — not things that happen once in a while. There’s no scoring and no “right” number of boxes. This simply helps us understand how they work so we can meet them where they are. Should take about 5–10 minutes. Either parent is welcome to complete this, together or separately.
